How much more do people eat when served a larger portion?

A Cochrane review of 58 studies and 6,603 participants found that people consumed more food when researchers offered larger portions, packages, units, or tableware. The result does not mean that one large serving increases every person’s intake by a fixed percentage.

The reviewers collected randomized trials that changed a food portion, package, individual unit, or item of tableware. They then compared how much participants selected or consumed. Across 86 food-consumption comparisons from 58 studies, participants exposed to larger sizes consumed more food.

Participants offered larger sizes consumed more food than participants offered smaller sizes.

The size offered to participants was associated with how much food they consumed.

The review compared average intake between randomized size conditions. It included trials in which researchers or food providers changed the amount in front of participants, not only situations in which participants deliberately ordered a larger serving.

The evidence therefore concerns the food environment as well as personal choice. It does not support blaming an individual for responding to the amount served.

The model does not predict how much one person will eat at one large meal.

The reviewers modeled what might happen if the observed effect persisted across an entire diet. Under that assumption, average daily food energy intake among UK children and adults could be 144–228 kcal, or 8.5–13.5%, lower after sustained reductions in exposure to larger sizes.

Most included studies were conducted in the United States and measured short-term intake. The review did not prove that serving a smaller initial portion at home produces the same difference over months or years.
